Direct Primary Care Notice NexVita Health provides medical services through a Direct Primary Care (DPC) membership model. Direct Primary Care is a healthcare arrangement in which patients pay a recurring membership fee for access to certain primary care services provided by the clinic. This membership is not health insurance and does not replace health insurance coverage. NexVita Health does not bill insurance for membership services and does not assume financial responsibility for healthcare services obtained outside this practice. Patients are strongly encouraged to maintain health insurance or other healthcare coverage to pay for services not provided by this clinic, including hospital care, specialty care, diagnostic imaging, and emergency services. The NexVita Health membership includes access to specific services as outlined in the membership agreement and service description. Services not included in the membership—such as certain laboratory testing, imaging studies, medications, specialty consultations, advanced diagnostics, or therapeutic services—may require additional fees. These services may be provided either by NexVita Health or by third-party providers and may be billed separately. Certain services offered by NexVita Health may include wellness, preventive, metabolic, or longevity-focused therapies that may not be covered by insurance and may not be considered medically necessary by insurance carriers. Participation in these services is voluntary and fees will be disclosed in advance. All medical recommendations, diagnostic testing, and treatments are provided at the clinical discretion of the medical provider based on each patient’s individual health status, medical history, and clinical judgment. Membership fees are billed on a recurring basis as described in the membership agreement. Membership fees, services, and policies may be modified from time to time. Patients may cancel their membership according to the terms outlined in the membership agreement. NexVita Health is a private medical practice and does not guarantee the availability of services outside normal clinic operations. Membership does not guarantee unlimited services or priority access to care beyond what is described in the membership agreement. Emergency Care NexVita Health does not provide emergency medical services. If you are experiencing a medical emergency, call 911 or seek immediate care at the nearest emergency department. Medicare Notice Patients who are enrolled in Centers for Medicare & Medicaid Services Medicare should be aware that services provided through a Direct Primary Care membership may not be reimbursable by Medicare. Patients are responsible for understanding how participation in a Direct Primary Care membership may interact with their Medicare benefits. Regulatory Notice In accordance with Colorado law governing Direct Primary Care agreements, this membership arrangement is not insurance and is not regulated as insurance by the State of Colorado.
