The Primary Care Intake is a comprehensive, two-part evaluation that establishes the foundation of your ongoing care. At NexVita Health, this process combines an in-depth history and physical with diagnostic testing and a follow-up visit to create your personalized care plan. Includes: • Comprehensive medical history and physical exam • Diagnostic lab work and clinical testing • EKG and CT coronary calcium scoring (as indicated) • Personalized care plan with follow-up and telehealth visits • Same or next-day appointments when available • Unlimited secure messaging with your provider • 10% discount on NexVita services and dispensary savings • Access to the NexVita Journal Pricing & Structure: $99 for the Comprehensive Intake + $449 per quarter for ongoing Direct Primary Care membership. Care Eligibility & Alignment: NexVita Health specializes in regenerative and longevity-focused care. Not all health concerns may be a fit. If we determine NexVita isn’t aligned with your needs, no fees will be assessed.
