The Primary Care Intake is a comprehensive, two-part evaluation designed to establish the foundation for your long-term care at NexVita Health. It allows us to understand your medical history, current concerns, and long-term health goals. Unlike traditional visits that focus on a single issue, the NexVita intake evaluates the full picture—developing a personalized strategy centered on prevention, metabolic health, and long-term wellness. This process includes an in-depth history and physical exam, baseline diagnostics, and a follow-up visit to review results and build your care plan. Your Intake Includes: Comprehensive Evaluation Personalized Care Planning + Follow-Up Baseline Laboratory Testing Baseline EKG Carotid IMT Ultrasound Abdominal Aortic Ultrasound Membership Benefits Additional testing or imaging may be recommended based on your needs. Any added services and costs will be discussed during your visit. As a Direct Primary Care model, your intake also begins an ongoing provider relationship—focused on personalized care, improved access, and proactive long-term health. Pricing & Structure: $99 Comprehensive Intake $449 per quarter ongoing membership Conditions We Manage: Primary care is the foundation of long-term health. NexVita Health evaluates and manages a wide range of conditions within a proactive, longevity-focused model, while coordinating referrals when needed. Care Eligibility & Alignment: NexVita Health focuses on regenerative and longevity-based care. If we determine the model isn’t the right fit, no fees will be assessed.
